Johnston County Emergency Services: Special Medical Needs Registry
Johnston County Emergency Services maintains a voluntary Special Medical Needs Registry to help emergency officials better plan for and respond to large-scale emergencies that may impact residents with certain medical needs.
This registry is designed to identify the locations and needs of residents who may require extra consideration during environmental emergencies such as hurricanes, tornadoes, ice storms, extended power outages, or other disasters.
Who Should Register?
A person with special medical needs is someone whose health requires daily medical attention and who may be adversely affected by the interruption of essential services, including:
- Utilities, such as electricity or water
- Transportation
- Daily professional care
- Medical equipment or treatment needs
- Other support needed to remain safely at home
This registry is intended for residents with major medical concerns that require daily medical attention. If you do not have medical needs that require daily care, you do not need to complete this registry form.

Why Register?
Information submitted through the Special Medical Needs Registry helps Johnston County Emergency Services maintain a better operating picture before, during, and after large emergencies.
Our goal is to support emergency planning efforts and help residents with special medical needs prepare to remain at home in a healthy and safe environment whenever possible.
By registering, you help emergency officials better understand where vulnerable residents may be located and what needs may exist during a disaster.
Important Information
The Special Medical Needs Registry is a planning tool. Registration does not guarantee assistance, priority response, transportation, shelter placement, or restoration of utilities during an emergency.
In an emergency, always call 911.
Residents and caregivers should still have an emergency plan, a supply kit, backup power options if needed for medical equipment, and a plan for transportation or evacuation if conditions become unsafe.
Permission to Share Information During Emergencies
By submitting the registration form, applicants agree to have their name added to the Johnston County Emergency Services Special Medical Needs Registry.
In the event of an emergency or disaster, applicants authorize the exchange of necessary information between emergency response agencies involved in the response. Applicants also grant emergency responders permission to enter the home, if necessary, to check on their safety and welfare.
