10 Johnston County Public Health Department Strategic Action Plan 2019 - 2021 Mission Statement Healthy Johns t onians in All Communities Vision Statement To provide quality healthcare , promote a safe environment , and partner with Johnstonians to foster he althy lifestyles Johnston County Strategic Plan This plan was adopted in December 20 19 with the objective to identify and set priorities for the operation of the Health Department for the next three years Priorities are based upon review and analysi s of ? Statistical data relative to the health status indicators of the Johnston County population ? Community surveys and assessment ? Experience and expertise of staff in identifying resources and needs ? Community Health Needs Assessment CHNA , Communi ty Health Improvement Plan s CHIP s , and State of the County Health SOTCH report The priority areas in this document are intended to outline fields of emphasis based on community needs These are not intended to exclude or discourage any existing or o ther program goals or interests The Health Director , Management staff , and Health Educators will continue to identify emerging issues that affect the community's health status The Health Department will pursue new initiatives based on other changes in th e county that affect health concerns and intervene with appropriate practice This plan will serve as a road map with modification , intervention , and evaluation as the ongoing methodology of operation Community Health Needs Assessment The 201 8 Johnst on County Community Health Needs Assessment CHNA is the H ealth D epartment's mechanism of assessing the County ' s health status by utilizing statistical data and obtaining knowledge of economic factors and social influences within the County This Assessm ent was developed through a partnership between the Johnston County Public Health Department , Johnston Health , Health ENC and Conduent Healthy Communities Institute Statistical data sets were obt ained from a variety of sources and analyzed through Conduen t HCI's community indicator database Primary data was obtained through a community survey distributed via focus group , online , and paper that reached over 1000 Johnston County residents This information was compared to North Carolina Counties and US co unties to identify a relative need , and weigh these areas of need against state and national values , trends over time , Healthy People 2020 targets , and Healthy North Carolina 2020 targets The reoccurring health priorities identified d uring the 2018 CHNA were in alphabetical order ? Access to Health Services ? Economy ? Exercise , Nutrition & Weight ? Heart Disease & Stroke ? Other Chronic DiseasesPage? Respiratory Diseases ? Social Environment ? Substance Abuse ? Transportation Health Priorities & Important Points in the Johnston County Community Action Plan Public health is the practice of disease detection and prevention in populations It is the science and art of protecting and improving the health of communities through educating providers and the public , promoting healthy lifestyles , and detecting and preventing the causes of disease and injury Public health works to prevent costly and complex health problems before they occur Supporting local public health supports economic development and potential for improveme nts in other areas that are important to community resilience Johnston County Health Department strives to meet the essential function of public health , which is the foundation of the Strategic Plan The above reoccurring health priorities were reviewed by a number of community partners , stakeholders and community members The final step in the Community Health Assessment process is a collaborative community effort using a series of meetings with community partners to set priorities and make recommendati ons for the develop ment and implementation of interventions as sociated with each health priority The prioritization process identified five focus areas 1 Access to Health Services 2 Mental Health Substance Abuse 3 Heart Disease & Stroke 4 Respiratory Disease 5 Transportation These priorities were chosen based on several criteria ? Johnston County ' s Health Rankings scored lower than the state and national percentages in these categories ? Healthy North Carolina 2020 objectives ? Facto rs contributing to the coun ty's morbidity and mortality rates ? Community partners and stakeholders , during collaborative meetings , identified these as significant issues within the county that could be addressed ? The Health Department and its partners have the personnel , resources , and expertise to support programs , policies , and procedures to make a positive impact on these priority areas ? Johnston County has less availability of primary care , mental health , and dental care providers compared to other counties in North Carolina , a nd has a high rate of preventable hospital stays Planning and recommendations were made based on the use of current data , organizational need , and contributions from community partnerships This strategic plan reflects the areas of focus of the Johnston County Public Health Department for the years 201 9 - 20 21 The Johnston County Health Department , along with its partners , is committed to implementing the plan to improve the health status of the residents of Johnston County Additionally , the JCPHD Ad ministrative Team strives to achieve the following guiding principles ? Local Health Department Accreditation Standards as defined in policies and procedures for all current programs Page? Expanded provision of services that are based on community need ? Quali ty services t hrough ong oing assessment and improvement ? Internal collaboration and interoperability with Health Department partners ? A qualified workforce ? A comprehensive preparedness response to public health emergencies ? Adequate physical facilities to meet community needs Strategic Goals & Interventions With the completion of the 2018 CHNA and identification of the five areas of focus , Community Health Improvement Plans CHIPs were fo rmulated around the goals and resources identified through the collaboration process Based on all availabl e information , seven interventions were formulated to assist county residents in developing healthy behaviors related to these five priority area s 2019 - 20 21 Interventions see formal CH IPs for detailed inf ormation These are the activities in which the JCPHD is the lead agency Ongoing information and outcomes will be detailed in the State of the County Report Priority identified Intervention Expected outcome Eval Method Outcome s Access to Health Servi ces Expand the Safety Net to include at least 1 new provider each year Increase the collaboration resources available to the county Activity of Safety Net providers See State of the County Report SOTCH Access to Health Services Every year , JCPHD will m entor 20 students Increase in number of DO Medical students receiving opportunities for preceptorships DO students will evaluate their experience Mental Health and Substance Abuse Opioid Task Force will host one Operation Medicine Drop event yearly In crease in knowledge relating to substance use and mental health Post surveys following each event to measure impact Heart Disease and Stroke Conduct Faithful Families program at local faith organizations Increase in attitudes and knowledge regarding heal thy eating Surveys will be completed by participants to measure impact Respiratory Diseases JCPHD will offer one evidence based smoking cessation program per year A decrease in the number of adults who use tobacco products Class attendance sheets , pre p ost tests conducted each class seriesPageRespiratory Diseases Refer COPD patients who smoke to Quitline program Increase the number of Quitline referrals of COPD patients Data gathered from Quitline staff Transportation Provide Mobile WIC services in 2 ad ditional municipalities Increase in enrollment and utilization of WIC services WIC utilization dataPage
