10 Johnston County Public Health Department Strategic Action Plan 2015 - 2019 Mission Statement Healthy Johnstonians in All Communities Vision Statement To provide quality healthcare , promote a safe environment , and partner with Johnstonians to foster healthy lifestyles Johnston County Strategic Plan This plan was adopted in September 2015 with the objective to identify and set priorities for the operation of the Health Department for the next four years Priorities are based upon review and analysis of • Statistical data relative to the health status indicators of the Johnston County population • Community surveys and assessment • Experience and expertise of staff in identifying resources and needs • Community Health Assessment and State of the County Health SOTCH report The areas of priority in this document are intended to outline areas of emphasis based on community needs These are not intended to exclude or discourage any existing or other program goals or interests The Health Director , Management staff and Health Educators will continue to identify emerging issues that affect the community’s health status The Health Department will pursue new initiatives based on other changes in the county that affect health concerns and intervene with appropriate practice This plan will serve as a road map with modification , intervention , and evaluation as the ongoing methodology of operation Johnston County Health Department strives to meet the essential function of public health , which is the foundation of the Strategic Plan Community Health Assessment The 2014 - 2018 Johnston County Community Health Assessment is the health department’s mechanism of assessing the County’s health status by utilizing the statistical data and obtaining knowledge of economic factors and social influences within the County Statistical data sets were obtained from a variety of sources , including US Census , State Data Center , NC Employment Security Commission , NC Department of Commerce , Federal Statistics , Centers for Disease Control and Prevention , National Institutes of Health , and survey of Johnston county residents The data from these various sources will be somewhat different based on the comparative analysis utilized However , the combined data allow a fairly close picture of the health needs and resources of the county residents The reoccurring health priorities identified by the Johnston County Residents during the 2014 - 2018 Community Assessment were in alphabetical order • Access to care including affordability of services and insurance , transportation , availability of providers • Cardiovascular disease • Dental care • Diabetes • Obesity and overweight • Mental health , particularly substance abuse services A review and analysis of Johnston County’s statistical data by the CHA Committee identified the following health issues as problematic by local , state and national benchmarks • Dental services for adults and children • Physical Inactivity School’s Curricula • Availability of Mental health and substance abuse services • Need for transportation and affordability to healthcare • Lack of sufficient navigators to assist with enrollment and provide insurance informationPageHealth Priorities & Important Points in the Johnston County Community Action Plan Johnston County Public Health Department’s objectives are to prevent illness , disease , and injury ; promote healthy living ; and keep the environment clean , healthy and safe The needs of the community were identified through the Johnston County Community Health Assessment Process , during which statistical information was analyzed from Johnston and peer counties by a number of community partners , stakeholders and community members It is important to assess health status behaviors , such as obesity , participation in health care and exercise , because they contribute to the health profile of the local community The final step in the Community Health Assessment process is a collaborative community effort using a series of meetings with community partners to set priorities and make recommendations for the development and implementation of interventions associated with each action plan The three health priorities in Johnston County that have been designated are access to health care both physical and behavioral health , prevention of obesity and overweight in our communities , and the importance of health nutrition choices and physical activity in our populations These priorities were chosen based on several criteria • Johnston County’s Health Rankings scored lower than the state percentages in these categories • Healthy Carolina 2020 objectives • Factors contributing to the counties ’ morbidity and mortality are behaviors such as lack of physical activity , poor nutrition and obesity • Access to care and overweight obesity were repeatedly identified during meetings as the top health issues in our county • Community partners and stakeholders , during collaborative meetings , identified these as significant issues within the county that could be addressed • The Health Department and its partners have the personnel , resources , and expertise to support programs , policies , and procedures to make a positive impact on these priority areas • Steady decline in the number of primary care physicians from 2009 - 2012 when data was reviewed Planning and recommendations are made based on the use of current data , organizational need and contributions from community partnerships This strategic plan reflects the areas of focus of the Johnston County Public Health Department for the years 2015 - 2018 The Johnston County Health Department , along with others , is committed to implementing the plan to improve the health status of the residents of Johnston County The Administrative Team of the Johnston County Health Department strives to achieve the following guiding principles • Local Health Department Accreditation Standards as defined in policies and procedures for all current programs • Expanded provision of services that are based on community need • Quality assurance program that assures quality service • Internal collaboration and interoperability with Health Department partners • A qualified workforce • Expanded service availability as necessary • A comprehensive preparedness response to public health emergencies • Adequate physical facilities to meet community needs Strategic Goals & Interventions Upon the completion of the 2014 - 2018 Community Health Assessment , three Action Plans were formulated based on local health data information in conjunction with local partners These three action plans focused on access to health care both physical and behavioral health , obesity and overweight citizens in our communities , and the importance of nutrition and physical activity These action plans further identified health disparity groups among the county residents low - income residents , uninsured individuals , low socioeconomic groups and minorities Based on all available information , 11 interventions were formulated to assist county residents in developing healthy behaviors related to these three areas Page2015 -
