10 Johnston County Public Health Department Strategic Action Plan 2022 - 202 4 Mission Statement To provide quality healthcare , promote a safe environment , and partner with the citizens of Johnston County to foster healthy lifestyles Vision Statement H ealthy Johnstonians in All Communities Johnston County Strategic Plan This plan was adopted in May 2022 with the objective to identify and set priorities for the operation of the Health Department for the next three years Priorities are based upon re view and analysis of • Statistical data relative to the health status indicators of the Johnston County population • Community surveys and assessment • Experience and expertise of staff in identifying resources and needs • Community Health Needs Assessment CHNA , Community Health Improvement Plan s CHIP s , and State of the County Health SOTCH report The priority areas in this document are intended to outline fields of emphasis based on community needs These are not intended to exclude or discourage a ny existing or other program goals or interests The Health Director , Management staff , and Health Educators will continue to identify emerging issues that affect the community's health status The Health Department will pursue new initiatives based on oth er changes in the county that affect health concerns and intervene with appropriate practice This plan will serve as a road map with modification , intervention , and evaluation as the ongoing methodology of operation Community Health Needs Assessment The 2022 Johnston County Community Health Needs Assessment CHNA is the H ealth D epartment's mechanism of assessing the County ' s health status by utilizing statistical data and obtaining knowledge of economic factors and social influences within the Count y This Assessment was developed through a partnership between the Johnston County Public Health Department , Johnston Health , Health ENC , Foundation Health Leadership and Innovation LHI , Brody School of Medicine , and Duke Endowment Statistical data se ts were obt ained from a variety of sources and analyzed through Conduent HCI's community indicator database Previously , these discussions and prioritization activities would have been conducted in person , but due to COVID - 19 restrictions , they were held via teleconference and SurveyMonkey Taking into consideration key stakeholder feedback & properly weighing secondary data sources , the JCPHD & Johnston Heath unanimously voted to adopt the following three health priorities for the 2021 - 2022 CHNA This i nformation was compared to North Carolina Counties and US counties to identify a relative need , and weigh these areas of need against state and national values , trends over time , Healthy People 2020 targets , and Healthy North Carolina 20 2 0 targets T he reoccurring health priorities identified d uring the 20 22 CHNA were in alphabetical order • Access to a primary care physician • Access to healthy foodsPage• Adult obesity • Adult smoking • Economic opportunity • Good parks and recreation facilities • Help during tim es of need • Lack of affordable housing • Low Income • Preventable hospital stays • Substance use misuse abuse • Transportation • Uninsured Health Priorities & Important Points in the Johnston County Community Action Plan Public health is the practice of disease det ection and prevention in populations It is the science and art of protecting and improving the health of communities through educating providers and the public , promoting healthy lifestyles , and detecting and preventing the causes of disease and injury P ublic health works to prevent costly and complex health problems before they occur Supporting local public health supports economic development and potential for improvements in other areas that are important to community resilience Johnston County Health Department strives to meet the essential function of public health , which is the foundation of the Strategic Plan The above reoccurring health priorities were reviewed by a number of community partners , stakeholders and community members The final st ep in the Community Health Assessment process is a collaborative community effort using a series of meetings with community partners to set priorities and make recommendations for the develop ment and implementation of interventions as sociated with each hea lth priority The prioritization process identified three focus areas 1 Access to Care 2 Substance Use Disorders 3 Transportation These priorities were chosen based on several criteria • Healthy Carolina 2020 objectives • Facto rs contributing to the county ' s morbidity and mortality rates • Community partners and stakeholders , during collaborative meetings , identified these as significant issues within the county that could be addressed • The Health Department and its partners have the personnel , resources , a nd expertise to support programs , policies , and procedures to make a positive impact on these priority areas • Johnston County has less availability of primary care , mental health , and dental care providers compared to other counties in North Carolina , and has a high rate of preventable hospital stays Planning and recommendations were made based on the use of current data , organizational need , and contributions from community partnerships This strategic plan reflects the areas of focus of the Johnston Co unty Public Health Department for the years 2022 - 2024 The Johnston County Health Department , along with its partners , is committed to implementing the plan to improve the health status of the residents of Johnston County Additionally , the JCPHD Admin istrative Team strives to achieve the following guiding principles Page• Local Health Department Accreditation Standards as defined in policies and procedures for all current programs • Expanded provision of services that are based on community need • Quality services t hrough ong oing assessment and improvement • Internal collaboration and interoperability with Health Department partners • A qualified workforce • A comprehensive preparedness response to public health emergencies • Adequate physical facilities to meet community needs Strategic Goals & Interventions With the completion of the 2022 CHNA and identification of the three areas of focus , Community Health Improvement Plans CHIPs were fo rmulated around the goals and resources identified through the collaboration process Based on all availabl e information , seven interventions were formulated to assist county residents in developing healthy behaviors related to these three priority area s 20 22 - 20 2 4 Interventions see formal CH IPs for detailed infor mation These are the activities in which the JCPHD is the lead agency Priority identified Intervention Expected outcome Eval Method Outcome s Access to Care Every year , JCPHD will mentor 20 students Increase in number of physicians and Advanced Practic e Providers receiving opportunities for preceptorships Students will evaluate their experience Ongoing Access to Care Determine deficits in primary care and specialty care by identifying providers that are aging out , no longer taking new patients , or lac k of medical services in the county Increase medical providers in Johnston County The primary evaluation will be the number of primary care doctors recruited or retained Secondary evaluation may include average duration of employment after recruitment , future intentions and cost Ongoing Substance Use Disorders Increase the number of doses of Narcan distributed in Johnston County Increase the number of lives saved due to drug overdose Evaluating monthly stats received from EMS of Johnston County Evalu ating location s of Narcan distribution within the county based on zip codes provided at pick up Ongoing - See State of the County Report SOTCH In 2022 , 1,140 doses of Narcan were distributed to various community partners and community members PageSubstance Use Disorders Increase the number of Quitline Referrals Provide education and resources to Johnston county residents Evaluating data received for Johnston County from the Regional Tobacco Prevention & Control Coordinator Ongoing - See State of t he County Report SOTCH The number of Quitline referrals increased by 329 individual referrals from 2020 - 2022 Substance Use Disorders Offer Evidence Based Smoking Cessation Class Offer an evidence based smoking cessation program at least once a year and for participants to complete the 8 - session program Evaluate the number of programs offered in a year's timeframe ; Evaluate the number of participants that completed the full 8 - week program Ongoing Transportation Johnston County residents will have acce ss to WIC through Tele - W IC Reduce the proportion of persons who are unable to obtain or are delay ed in obtaining necessary medical care and nutrition assistance due to lack of transportation Evaluate the number of clients that have been utilizing the Tel e - WIC service Evaluate how many clients that continue to renew by phone as opposed to doing so in person Ongoing - See State of the County Report SOTCH The number of residents served between 2021 and 2022 increased by 12 In 2021 , WIC served 4,680 cli ents a month , and in 2022 , they served 5,260 clients a month Transportation Increase the number of Women , Infant and Children satellite clinics in Johnston County Reduce the proportion of persons who are unable to obtain or are delay ed in obtaining neces sary medical care and nutrition assistance due to lack of transportation Evaluate the number of clients utilizing this service once it resumes Ongoing - Resuming opening the satellite clinics have been delayed due to the Covid - 19 Pandemic The 3 sate llite sites in Benson , Clayton and Princeton still exist with plans to resume services in 2024 Page
