Johnston County Veterans Services — Transportation Assistance

Complete all required fields below to request transportation to an appointment. After you submit, the Veterans Services Office reviews your request and contacts you to confirm.

RIDER REQUIREMENTS & RIDE TIMELINE DISCLAIMER

Requirements:

Veterans must submit this transportation request at least 5 business days prior to their scheduled appointment.
Rides are strictly for transportation to and from VA Medical Facilities or authorized Community Care locations.
Out-of-county transportation is only available on Mondays and Wednesdays.
Riders must be ready at the designated pickup location at the exact time provided in the final confirmation email.

Timeline of Ride Disclaimer:

Submitting this form does NOT instantly confirm your ride. This is an initial request.
Once submitted, a Veteran Service Officer will review your form and forward it to JCATS Dispatch for official scheduling.
JCATS will approve the request and assign a specific pickup time based on their routing. You will receive a final confirmation email containing your approved pickup time once JCATS has officially placed you on the schedule.
