1. Online, through our office (recommended). Fill out the form below. A Veterans Services Officer reviews, certifies, and signs your application, then produces the official MVR-33A.

2. On your own. Download the official MVR-33A, bring it to a Veterans Service Officer for certification, then submit it to the NC Division of Motor Vehicles.
